Well, obviously we love In-N-Out. But out here on the East Coast we have a place called Five Guys that is equally as yummy, and if not for the higher prices, might be even yummier. They have great burgers, lots of toppings, and cajun fries. The inside decor is red and white like In-N-Out.
If you come to my neck of the woods...other than the in-n-out... Squeeze Inn... it was featured on Guy Fieri's Diners, Drive-ins and Dives Food Network show...and it is ... worth it. ;-)
I agree- In-N-Out. And their milkshakes rock. :) Or if you're looking for slightly more upscale- Islands. Yum! Their Bleu-nami rocks! (bleu cheese burger)
Obviously both of those are more pricey than In-N-Out.
There is also a place in in Utah (I think Lehi). I will have to ask my sister for the name, but it was fantastic! The burgers were good and the shakes completely blew In-N-Out out of the water. They were more ice cream than shake, but they were excellent. My brother-in-law said that this place is THE dinner spot after Priesthood Conference sessions.

I would second Islands and add Red Robin.
